Follow-Up after Hospitalization for Mental Illness

What are we tracking and why?

We track patients receiving timely follow-up care after being hospitalized for certain mental health disorders. Follow-up appointments help patients build upon progress made during hospitalization, and help ease the transition back home. During these appointments, prescribed medications can be discussed, and adjustments can be made if needed.

Regardless of where hospitalization takes place (here, another military treatment facility, or a civilian hospital), all TRICARE Prime beneficiaries enrolled to our medical center are included in this measure. We track the following rates:

1. The percentage of patients who received follow-up within 7 days of discharge.
2. The percentage of patients who received follow-up within 30 days of discharge.

How are we doing?

What are we doing to improve?

  • We diligently work to identify our TRICARE Prime enrollees who have started inpatient behavioral health treatment, even if that treatment is taking place at another medical facility.
  • We collaborate with our patients’ case managers and discharge planners to coordinate follow-up appointments after they are discharged from the hospital.

What can you do?

If you or a loved one is hospitalized, remember to schedule a follow-up appointment before you leave the hospital. Try your best to continue to schedule subsequent follow-ups at the end of each appointment.

If you, or a dependent, have recently been hospitalized for a mental illness, please contact one of our behavioral health clinics to schedule a follow-up appointment. Be sure to tell us that you are scheduling a follow-up appointment after being hospitalized.

If you have missed a follow-up appointment, please reschedule it as soon as possible.

Make sure your contact information is up-to-date with our Patient Administration Division and in DEERS (Defense Enrollment Eligibility Reporting System: (DEERS) A database of information on uniformed services members (sponsors), U.S.-sponsored foreign military, DoD and uniformed services civilians, other personnel as directed by the DoD, and their family members. You need to register in DEERS to get TRICARE.). Keeping your information current allows your doctor to provide better management of your health care.
