Refer a Patient

Providers have the option to refer patients to the Intensive Outpatient Program (IOP) or the TBI Service Outpatient Clinic.

NICoE Intensive Outpatient Program

The primary care provider from the patient’s home care team must submit IOP referrals through the NICoE Referral site, located in the TBI Portal on the DHA CarePoint Information Portal. The NICoE Referral site can be accessed here.

When opening the NICoE Referral site a pop up box labeled “Select a Certificate” will appear; your DOD EMAIL certificate must be selected to enter the site.

NICoE TBI Service Outpatient Clinic

The NICoE's Referral Management Team will determine appropriate candidates for the TBI Service Outpatient Clinic upon reviewing referrals.
Within the National Capital Region:
Provider referrals may be entered directly into the Composite Health Care System (CHCS)
Outside of the National Capital Region:
