For Patients Enrolled to the Pediatric or Adolescent and Young Adult Medicine Clinic, and their Caregivers
The Disease Management Service for pediatric, adolescent and young adult patients is led by an experienced nurse educator who provides personalized health education to patients ages 0 to 26, and their families. The most commonly discussed topics include:
- Asthma
- Spacer Training
- Weight Management
- EpiPen Use
- Constipation
- Sexual Health
- And More!
Every appointment is customized to meet your individual needs, and gives you ample time to discuss your health care concerns and questions. Sessions may include:
- An overview of your health condition
- Learning self-management techniques
- Setting personal health goals
- Creating an action plan to help you achieve your health goals
