Adult Disease Management Service
For Patients Enrolled to the Internal Medicine Clinic who have a Chronic Medical Condition
If you would like help managing your chronic health condition, or simply want to learn more about it, then our Adult Disease Management Service may be a benefit to you and your family.
Led by an experienced nurse educator, the Adult Disease Management Service provides counseling and support sessions to patients with chronic conditions such as diabetes, hypertension, dyslipidemia, polycystic ovarian disease (PCOS), heart failure, Parkinson’s disease, pre-diabetes, weight management issues, and more.
Each session is customized to meet your individual needs and may include:
- An overview of your health condition
- Learning self-management techniques
- Setting personal health goals
- Creating an action plan to help you achieve your health goals
Please Note: The Adult Disease Management Service does not provide medication management. However, periodic medication reviews are recommended for patients over 65 years old, and those with multiple medications or complex medical conditions.
