Medication Monitoring & Assistance Programs
Sole Provider (SP): The only provider authorized to prescribe or countersign controlled medication prescriptions for a specified patient requiring chronic therapy. Recorded in medical record; signs Controlled Substance Agreement.
Purpose - To delineate the policy and procedure for the identification and management of patients exhibiting signs of drug-seeking behavior, insufficient analgesia, evidence of non-optimization in care options, psychosocial issues, or other complex pharmaceutical care issues.
Applicability - To all military and civilian personnel assigned or attached to WRNMMC. Applies to outpatients (inpatients should be identified during medication reconciliation process for discharge planning)
Naloxone Rescue Program - This program is for high risk patients that are prescribed opioids, the naloxone has been demonstrated to be life saving if administered during overdose.
