Advance Care Planning

We're committed to assisting patients and families in planning for future healthcare decisions. Planning can be challenging, so it’s important to speak openly about facing decisions that have significant impact on how you live your life. This can be a time for profound personal growth, and to reevaluate what is truly important in your life.

The Oncology Support Team can help facilitate these important discussions, which may include advance directives for children, adolescents and adults, and the role of having a Power of Attorney. In-patient support and in-home hospice support may also be available.
